Job summary

Transition Projects, Inc. is seeking a Veterans Housing Case Manager I to support housing placement and sustainment for participants in Portland.

This full-time, on-site role focuses on engaging participants, reducing barriers to housing stability, and coordinating services within the community. The ideal candidate will have a related bachelor’s degree or equivalent social services experience, knowledge of mental health issues, and strong collaboration with agencies.

Qualifications

• Bachelor's Degree in a related field is required or 2 years of social services experience.
• Demonstrated ability to interact with other agencies and advocate for participants.
• Knowledge of chemical dependency and mental health issues and resources.
• Ability to build positive relationships with property/building managers.
• Mediation between agencies, participants, and managers.
• Up-to-date knowledge of community resources for the homeless and veterans.

Responsibilities

• Develop and implement goals to attain housing stability and self-sufficiency.
• Address barriers including mental health, substance use, income development, and education.
• Coordinate and deliver services; meet with participants to ensure service delivery.
• Maintain participant files and HMIS documentation; adhere to budget guidelines.
• Provide monthly case management reports and lead engagement groups.
• Attend staff and partner meetings and coordinate with agency staff.
