Job summary

Wounded Warrior Project (WWP) is seeking a Complex Case Coordination Liaison to manage caseloads of warriors and coordinate complex mental health care with internal teams and external providers. You will assess needs, develop treatment plans, and advocate for rapid, coordinated access to resources.

You will collaborate with VA, DoD, and community partners while maintaining confidential documentation and supporting recovery and well-being for veterans and their families.

Qualifications

• Four years of experience in social work or mental health field.
• Four years of experience with veteran healthcare-focused programs and case management.
• Experience with military and veterans' population in support of development and readjustment.

Responsibilities

• Serve as subject-matter expert in planning, coordination, and execution of mental health programs.
• Maintain a caseload of assigned warriors.
• Perform screenings and needs assessments to coordinate external care.
• Develop and implement treatment plans with care teams.
• Coordinate with DoD, VA and community resources to ensure comprehensive warrior care.
• Ensure documentation in internal systems and data accuracy.
