Job summary

Wounded Warrior Project (WWP) is seeking a Complex Case Coordination Liaison to manage caseloads of warriors, coordinate mental health services, and collaborate with VA, private providers, and internal teams. The role focuses on crisis response, treatment planning, and reducing barriers to care.

The ideal candidate has four years of social work or related mental health experience with veterans and is prepared to obtain ASIST within a year.

Qualifications

• Four years of experience as a social worker or in a mental health related field.
• Four years of experience working with veteran healthcare-focused programs and case management.
• Four years of experience working with military and veterans' population in support of their development and readjustment.
• Bachelor's degree in social work, psychology, rehabilitation counseling, case management, or related mental health field.
• Master's degree in social work or related field (preferred).
• Ability to obtain ASIST Certification within 1 year of hire.

Responsibilities

• Serve as subject-matter expert and resource in planning, coordination, and execution of mental health programs and caseload management.
• Maintain a caseload of assigned warriors and coordinate care with clinicians and internal teams.
• Perform screenings, needs assessments, and coordinate external care.
• Develop treatment plans with care teams and empower warriors to participate in decisions.
• Coordinate with families and advocates to ensure post-discharge services and resources.
• Ensure accurate documentation in WWP databases and data systems.
