Job summary

Wounded Warrior Project (WWP) is seeking a Complex Case Coordination Liaison to respond to crisis referrals and guide warriors toward internal and external resources. You will manage treatment planning, advocate for rapid care, and coordinate with local and federal agencies to establish comprehensive plans for warriors in need.

Applicants should have four years of experience in social work or mental health, plus experience with veteran-focused programs.

Qualifications

• Four years of experience in social work or mental health field.
• Four years of experience with veteran healthcare-focused programs and case management.
• Proven ability to coordinate with DoD, VBA, VHA and community resources.
• Experience with coordinating care for warriors in crisis.

Responsibilities

• Serve as SME in planning and execution of mental health programs.
• Maintain caseloads and conduct needs assessments.
• Coordinate with care teams to implement treatment plans.
• Advocate for warriors to obtain necessary care and resources.
• Ensure accurate documentation and data management.
• Coordinate with internal/external resources for admissions and discharges.
