Job summary

Wounded Warrior Project (WWP) seeks a Complex Case Coordination Liaison to manage caseloads, coordinate mental health care for warriors, and collaborate with DoD, VBA, VHA and external providers. You will document care plans, support transition and connect warriors with WWP resources.

Requirements include a social work or mental health background with veteran-focused experience; ASIST certification within 1 year is preferred. Hybrid work options available.

Qualifications

• Four years of experience as a social worker or in a mental health related field.
• Four years of experience with veteran healthcare-focused programs and case management.
• Four years of experience working with military and veterans' population.

Responsibilities

• Caseload management and coordination of mental health services.
• Perform screenings and needs assessments for complex cases.
• Develop and implement treatment plans with care teams.
• Coordinate with DoD, VBA, VHA and external providers for warrior care.
• Maintain accurate documentation and data in internal systems.
• Provide transition support and program referrals to WWP services.
