Job summary

Wounded Warrior Project (WWP) is seeking a Complex Case Coordination (C3) Liaison to support crisis referrals, assessments, and care coordination for warriors. You will advocate for rapid, coordinated responses and coordinate with VA, private facilities, and community resources to ensure timely care.

You will manage caseloads, conduct needs assessments, and collaborate with care teams to implement warrior-centered treatment plans and documentation across WWP systems.

Qualifications

• Bachelor's degree in social work or related field is required.
• Experience with veteran-focused programs is preferred.
• Ability to obtain ASIST Certification within 1 year of hire.

Responsibilities

• Serve as subject-matter expert in planning, coordination, and execution of mental health programs.
• Maintain a caseload of assigned warriors and coordinate care.
• Perform screenings and needs assessments to identify external care.
• Develop treatment plans with care teams and document progress.
