Job summary

Wounded Warrior Project is seeking a Complex Case Coordination (C3) Liaison to respond to crisis referrals, assess needs, and guide warriors toward internal and external resources. You will coordinate with VA, DoD, and community partners to develop comprehensive care plans while supporting warriors and their families through treatment and recovery.

The role requires multidisciplinary collaboration, strong documentation, and a commitment to ethical care.

Qualifications

• Four years of experience as a social worker or in a mental health-related field.
• Four years of experience with veteran healthcare-focused programs and case management.
• Four years of experience working with military and veterans' population in support of their development and readjustment.

Responsibilities

• Serve as subject-matter expert in planning, coordination, and execution of mental health programs.
• Maintain a caseload of assigned warriors and perform needs assessments.
• Coordinate with care teams to develop and implement treatment plans.
• Develop warrior-centric relationships to support informed decisions and ongoing care.
• Document and protect sensitive health information per standards; coordinate admissions and discharges.
