Job summary

Wounded Warrior Project seeks a Complex Case Coordination (C3) Liaison to respond to crisis referrals, perform initial assessments, and guide warriors toward internal and external resources. The role acts as a case manager for treatment planning and removal of barriers to care.

Responsibilities include caseload management, coordinating with DoD/VBA/VA and private facilities, and ensuring documentation accuracy. A strong teamwork mindset and experience with mental health resources are essential.

Qualifications

• Bachelor's degree in a mental health field or related discipline is required.
• Four years of experience in social work or mental health, with veterans programs preferred.
• ASIST certification within 1 year of hire is preferred.

Responsibilities

• Maintain a caseload of warriors and coordinate comprehensive assessments.
• Coordinate with internal and external resources (VA, DoD, VBA, community services) to establish care plans.
• Document care activities and maintain data in WWP systems.
• Offer transition support and program referrals to WWP services.
