Job summary

Wounded Warrior Project (WWP) is seeking a Complex Case Coordination (C3) Liaison to manage caseloads of veterans and coordinate comprehensive mental health services. The role focuses on crisis referrals, initial assessments, shaping treatment plans, and advocating with local and federal resources to ensure timely care.

The position emphasizes collaboration with VA, DoD, and community providers, aligning with WWP's mission and ensuring warriors receive needed support and resources.

Qualifications

• Bachelor's degree in a related field is required or equivalent experience.

Responsibilities

• Serve as subject-matter expert in planning, coordination, and execution of mental health programs and resiliency initiatives.
• Maintain a caseload of assigned warriors and conduct needs assessments to coordinate external care.
• Work with care teams to develop and implement treatment plans aligned with warrior goals.
• Coordinate with families and advocates to ensure post-discharge services are provided.
• Enter data in WWP systems and ensure accurate case documentation.
