24 Hour Home Care is seeking a Community Supports Coordinator, Utilization Management in El Segundo, CA. • 1–3 years of customer service or related experience.
• Experience monitoring utilization and coordinating reauthorizations.
• Experience using Salesforce or similar CRM/case management systems.

Responsabilités

• Monitor member utilization of authorized services and identify underutilization trends.
• Coordinate reauthorization activities, tracking expirations and gathering docs.
• Serve as a primary contact for members and caregivers, addressing inquiries and service concerns.
• Collaborate with internal teams, health plans and partners to resolve authorization discrepancies.
• Maintain accurate records in Salesforce in HIPAA-compliant manner.
• Hybrid position requiring in-office presence 1x per month in El Segundo.
