Job summary

24 Hour Home Care is seeking a Community Supports Coordinator in El Segundo to oversee the member journey after admission, focusing on utilization, reauthorization, and care coordination with funding partners and health plans.

You will engage with members and caregivers, manage authorizations in Salesforce, and ensure timely service delivery while maintaining HIPAA compliance. This temporary hybrid role supports ongoing service continuity.

Qualifications

• High school diploma or GED required; bachelor’s degree preferred.
• 1–3+ years of customer service or other service-oriented experience.
• Experience with ticket management systems and processes.
• Proficiency with Microsoft Office, including Word, Excel, and Outlook, and experience with Salesforce.
• Bilingual Spanish preferred.
• Reliable internet service required.

Responsibilities

• Monitor member utilization and hours each billing cycle to address underutilization, overages, or service disruptions.
• Manage utilization and reauthorization cases in Salesforce with required documentation.
• Coordinate with funding sources, health plans, and Case Managers to resolve authorization issues.
• Build relationships with members and caregivers with proactive outreach and support.
• Respond to inquiries via phone, chat, text, and email with timely resolutions.
• Maintain accurate member records including utilization and authorization status.
• Collaborate with internal teams and external partners to resolve service issues while ensuring HIPAA compliance.
