Job summary

Omega Home Health Care Services is seeking a compassionate Physical Therapist to evaluate and provide therapy to patients in their homes as prescribed by the attending physician. The role involves collaborating with physicians, instructing patients in therapeutic activities, and documenting progress and outcomes.

Responsibilities include assessment, treatment planning, patient education for prosthetic/orthotic devices, and coordinating discharge planning with the care team while ensuring safety

Qualifications

• Graduate of a physical therapy school approved by the appropriate board.
• The American Physical Therapy Association approval or equivalent collaboration with the AMA Council of Medical Education.
• California state licensure or registration required.
• At least one year of institutional experience preferred.
• Valid CPR certification, driver's license, and auto insurance.

Responsibilities

• Evaluate patients in collaboration with the physician using diagnostic and prognostic tests.
• Instruct patients in prescribed therapies to relieve pain and restore function.
• Record patient responses and changes in condition to the physician and home health staff.
• Educate patients and families in use of prosthetic, orthotic, and assistive devices.
• Develop and document assessment, plan of care, and progress notes.
