Job summary

Omega Home Health Care Services Inc. is seeking a skilled Physical Therapist to evaluate and treat patients in home health settings. You will collaborate with physicians, design plans of care, and educate patients and families on maintaining function and reducing pain.

Travel to patients’ residences is required, with compensation per visit, evaluation, and admission according to agency rates. Requirements include CA licensure, CPR, a valid driver's license and insurance, and at least one year of

Qualifications

Graduate of a physical therapy program.
Licensed or registered by the state of California.
CPR certification, driver's license and auto insurance.
At least one year of institutional experience.
Ability to travel to prospective clients' residences.

Responsibilities

Evaluate patients with diagnostic and prognostic tests and set goals.
Guide patients in therapeutic activities to relieve pain and restore function.
Record patient responses and changes in condition with the physician.
Train patients and caregivers in prosthetic/orthotic devices and assistive devices.
Prepare assessment, plan of care and progress notes for the patient record.
Participate in discharge planning and complete discharge summaries.
