Job summary

Omega Home Health Care Services, Inc. is seeking a compassionate Physical Therapist to evaluate and provide physical therapy services to patients as prescribed by the attending physician.

The role involves traveling to patients’ residences to deliver care within a home health setting. Requirements include graduation from an approved physical therapy program, California licensure, and CPR certification, with a preference for at least one year of institutional experience.

Qualifications

• Graduate of a physical therapy school approved by the American Physical Therapy Association or equivalent.
• Licensed or registered by the state of California.
• Preference of at least one year of institutional experience.

Responsibilities

• Evaluate patients and apply diagnostic and prognostic tests to establish goals.
• Instruct patients in prescribed therapeutic activities to relieve pain and restore function.
• Record patient responses and changes to the therapy program to physicians and staff.
• Train patients and caregivers in the use of prosthetic, orthotic, and assistive devices.
• Prepare assessment, plan of care, and progress notes and integrate into the patient record.
• Prepare periodic evaluations and discharge summaries in line with agency policies.
• Communicate effectively with all care providers and report to physician and nursing staff.
• Discharge planning participation and completion of discharge summaries.
