Job summary

Omega Home Health Care Services, Inc. is seeking a compassionate Physical Therapist to evaluate patients and provide therapy services prescribed by physicians in a home health setting in California.

The role requires CA licensure, CPR certification, and valid driver’s license with the ability to travel to patients' residences. Typically 1 year of institutional experience is preferred, and the incumbent will perform assessments, develop care plans, and document patient progress.

Qualifications

• Graduate of a physical therapy program approved by major associations.
• Licensed or registered by the state of California.
• Preference of at least one year of institutional experience.
• Valid CPR certification, driver's license, and auto insurance.

Responsibilities

• Evaluate patients by applying diagnostic and prognostic tests and set goals with the physician.
• Instruct patients in therapeutic activities to relieve pain and restore function.
• Record and report patient responses and changes to physician and staff.
• Train patients and caregivers in the use of prosthetic, orthotic, and assistive devices.
• Prepare assessments, plans of care, progress notes, and discharge summaries.
• Coordinate discharge planning and complete discharge summaries per agency policies.
• Follow Plan of Care and communicate with all care providers.
• Notify the agency of absences affecting services.
