Job summary

Omega Home Health Care Services is seeking a compassionate Physical Therapist to evaluate and provide therapy to patients in their homes as prescribed by the attending physician.

The role requires a PT license in California, CPR certification, and strong communication skills. Experience is preferred and travel to client residences is expected; compensation is per visit and per evaluation/admission with rates specified by visit type.

Qualifications

• Must be a graduate of an approved physical therapy program and licensed or registered in California.
• CPR certification and valid driver’s license required.
• Preference for at least 1 year of institutional experience.

Responsibilities

• Evaluate patients with physician guidance using motor, nerve, joint and functional tests; set goals based on current function and potential.
• Instruct patients in therapeutic activities to relieve pain and restore function.
• Document patient responses and changes to therapy with physician and home health staff.
• Train patients and caregivers in use of prosthetic/orthotic devices and proper home therapy techniques.
• Prepare assessments, plans of care and progress notes; maintain patient records.
• Provide periodic evaluations and discharge summaries; follow Plan of Care and communicate with care team.
• Notify agency of absences; participate in discharge planning with staff and physicians.
