Qualifications

• Graduate of a physical therapy school approved by relevant authorities.
• California license required for practice.
• CPR certification and valid driver's license are required.
• Preference for at least one year of institutional experience.

Responsibilities

• Evaluate patients with diagnostic tests and establish goals based on function.
• Guide patients in prescribed activities to relieve pain and restore function.
• Record patient responses and report to physician and home health staff.
• Train patients, families, and caregivers in prosthetic/orthotic device use.
• Provide instructional materials and supervise the therapy program.
• Prepare assessment, plan of care, and progress notes for the patient record.
• Prepare periodic evaluations and discharge summaries as required.
• Follow the Plan of Care and communicate with care team.
• Notify agency of absences and participate in discharge planning.
• Complete discharge summaries within agency policies.
