Qualifications

• Graduate of an approved physical therapy school.
• Licensed or registered in California.
• 1 year of institutional experience preferred.
• CPR certification, valid driver's license and auto insurance.

Responsibilities

• Partner with the physician in evaluating patients by applying diagnostic and prognostic tests and establishing goals based on current function.
• Instruct patients in prescribed therapeutic activities to relieve pain and restore function.
• Record and report patient responses and any changes to the physician and home health office staff.
• Train patients, families and caregivers in use of prosthetic, orthotic and assistive devices.
• Prepare assessments, care plans and progress notes and incorporate them into the patient's record.
• Prepare periodic evaluations and discharge summaries as per agency policies.
• Follow the Plan of Care and communicate effectively with care teams.
• Notify the agency of absences due to illness or emergencies and reschedule services as needed.
• Participate in discharge planning activities with staff, patients and physicians.
