Job summary

Omega Home Health Care Services is seeking a compassionate Physical Therapist to evaluate patients and provide therapy per physician orders in patients' homes in California. The role requires California PT licensure, a valid CPR certification, and driving to client residences.

Responsibilities include evaluating, guiding therapeutic activities, documenting progress, and coordinating with physicians and staff. Travel to prospective clients’ homes is expected as part of the role.

Qualifications

• Must be licensed or registered by the state of California.
• Preference of at least one year of institutional experience.
• CPR certification, driver's license, and auto insurance are required.

Responsibilities

• Evaluate patients in collaboration with the physician and set goals based on current functioning.
• Instruct patients in therapies to relieve pain and restore function.
• Record patient responses and communicate changes to the physician and home health team.
• Train patients and families in use of prosthetic/orthotic devices and assistive devices.
• Outline and document the plan of care and progress notes in the patient record.
• Prepare evaluations and summary reports of the patient condition.
• Follow the Plan of Care and coordinate discharge planning with staff and physicians.
• Communicate effectively with all care providers and notify agency of absences.
