Job summary

Care Lync is seeking a dedicated Support Coordinator to manage casework for adults with IDD, autism, and physical disabilities within the Medicaid Waiver framework. You will develop individualized care plans, coordinate services, and monitor outcomes to ensure professional, cost-effective support in home and community settings.

The role emphasizes cross-functional collaboration, confidentiality, and ongoing training, with travel to participant locations up to half the time.

Qualifications

Requires a BS/BA degree in a related field.
Experience with Microsoft Office and state-maintained databases.
Must maintain proficiency in company sponsored trainings and certifications.
Must maintain state related Clearances.
Valid Driver's License with personal vehicle insurance and current vehicle registration.

Responsibilities

Complete assessments to identify participant needs and desired services.
Develop a Person-Centered Service Plan and update annually or with major changes.
Inform and involve participants in provider selection for fully informed decisions.
Ensure integration of Behavioral Support Plan with ISP where applicable.
Report incidents of abuse, neglect or exploitation to supervisor immediately.
Coordinate with other care coordinators and entities to ensure seamless service delivery.
Research and develop alternative solutions to participant needs.
Maintain confidentiality of sensitive health information.
Attend trainings and recertification sessions.
Travel ~50% to participants as needed.
