Zusammenfassung

Care Lync is seeking a dedicated Support Coordinator to develop individualized care plans for adults with IDD, disabilities, and autism enrolled in Medicaid Waiver programs. You will broker services, monitor ISP implementation, and ensure cost-effective, person-centered care in home and community settings.

Responsibilities include assessments, care planning, coordinating with providers, protecting confidentiality, and reporting incidents. Travel up to 50% is required to serve participants.

Qualifikationen

• Bachelor's degree or equivalent in a related field.
• Experience with Microsoft Office and state-maintained databases.
• Maintain CPR/First Aid certification and required trainings.

Aufgaben

• Complete assessments to identify participant needs and desires for service delivery.
• Develop a Person-Centered Service Plan and update annually or with changes.
• Coordinate with providers to ensure informed participant choices and ISP alignment.
• Ensure Behavioral Support Plan and Crisis Plan align with ISP.
• Report incidents of abuse, neglect, or exploitation immediately.
• Coordinate with care coordinators and other services for seamless delivery.
• Research and develop alternative solutions to participant needs.
• Maintain confidentiality of sensitive information.
• Attend required trainings and recertifications.
• Consult with supervisor to consider participant needs and limitations.
• Travel to participants approximately 50% of the time.
