Covenant House Missouri is a faith-based organization serving youth ages 16 to 24 who are runaway, trafficked, at-risk, or homeless in St. Louis.

The Case Manager delivers goal-oriented support, assessment-driven planning, and direct care to help youth reach independent, sustainable futures. Responsibilities include intake, crisis intervention, coordinating with an interdisciplinary team, providing life skills training, scheduling activities, and maintaining documentation per agency policies.

Qualifications

• Bachelor's degree in Social Work or related field, or equivalent experience.
• Master's degree preferred.
• Minimum two years’ experience preferred.

Responsibilities

• Establish and maintain professional boundaries with youth.
• Conduct intake and administer assessments to guide progress.
• Develop individualized treatment plans aligned with client goals.
• Provide crisis intervention counseling to youth and families.
• Coordinate with interdisciplinary team to plan and deliver services.
• Transport youth to appointments as needed.
• Maintain accurate documentation and comply with policies.
• Lead meetings and coordinate team communication.
• Administer medication per guidelines.
