Job summary

YWCA of St. Joseph is seeking a Community Case Manager to provide trauma-informed, survivor-centered case management for individuals pursuing housing services. You will assess needs, set goals, coordinate referrals, and maintain essential data and confidentiality while supporting housing stability.

You will travel between Bliss location and the main YWCA St. Joseph building to support program operations, attend trainings, and facilitate groups.

Qualifications

• Bachelor’s degree in human service field or equivalent education/experience.
• Knowledge of domestic & sexual violence dynamics preferred.
• Knowledge of community resources preferred.
• Excellent interpersonal and communication skills.
• Ability to maintain confidentiality.
• Ability to engage and build relationships with diverse populations.
• Access to private transportation, valid driver’s license with insurance per agency standards.

Responsibilities

• Provides trauma-informed, survivor-centered case management services for housing seekers.
• Facilitates assessments, goal setting, safety planning, and updates as needed.
• Develops and maintains relationships with community members and providers; coordinates referrals.
• Maintains knowledge of housing funding standards, guidelines, and contracts.
• Provides transportation to access services; ensures driving eligibility and insurance.
• Maintains client confidentiality per laws and policies.
• Ensures accurate data entry and timely documentation per funder requirements.
• Develops and facilitates educational and supportive groups.
• Attends staff meetings and trainings; supports YWCA mission to empower women.
• Other duties as required.
