YWCA St. Joseph is seeking a Community Case Manager to provide trauma-informed, survivor-centered housing services. You will assess needs, set goals, connect clients with resources, and support housing stability.

The role requires collaborating with partners, maintaining confidentiality, managing data, and occasionally providing transportation. Flexibility for evenings/weekends may be needed to meet guests' needs and program requirements.

Qualificações

• Bachelor’s degree in human service field or equivalent education/experience with related case management experience preferred.
• Knowledge of domestic & sexual violence dynamics preferred.
• Knowledge of community resources preferred.
• Excellent interpersonal and communication skills.
• Ability to maintain confidentiality.
• Access to private transportation, valid driver’s license with insurance complying with minimum agency standards.

Responsabilidades

• Provides trauma-informed, survivor-centered case management services according to Missouri Coalition Against Domestic and Sexual Violence (MOCADSV) service standards.
• Facilitates assessments, goal setting, safety planning, and other necessary evaluations, reviews, and updates as needed.
• Develops and maintains professional relationships with community members, partners, service providers, and other professionals.
• Maintains familiarity with all housing funding standards, guidelines, and contracts applicable to the program.
• Maintains knowledge of other YWCA housing programs and services to assist with appropriate referrals and coordination.
• Provides transportation as needed to assist clients in accessing services and community resources.
