Willoughby Western Lake County Chamber of Commerce is seeking a Patient Navigator-Behavioral Health to join the Crossroads Health team in Northeast Ohio. The role focuses on trauma-informed patient engagement, assisting with intake documentation and coordinating care for clients and their families.

The navigator will work closely with treatment providers, primarily handling phone and office-based interactions to ensure timely access to behavioral health services.

Qualifications

• Trauma-informed care approach in client interactions.
• Experience guiding clients through intake and care coordination.
• Strong communication with clients and treatment providers.
• Prior experience in a medical office with patient interaction is preferred.

Responsabilités

• Provide trauma-informed services to clients primarily on the phone and in the office.
• Assist clients in completing initial required documentation.
• Provide trauma-informed screening, triage and care coordination for clients and their families.
• Facilitate communication among treatment providers and clients to ensure engagement and access to care.
