Job summary

NorthCare seeks an outpatient care navigator to provide integrated services for adults, children, and families in a community setting. You will work toward a continuum of care that combines physical and behavioral health, emphasizing recovery, trauma‑informed care, and patient‑centered approaches.

Responsibilities include assessments, care planning, linking to housing, transportation, education, employment, and benefits, plus crisis management when needed.

Qualifications

• High school diploma or GED with 36 months direct consumer care in mental health/substance use.
• Alternatively 60 college credits with 12 months direct consumer care experience.
• Bachelor’s or Master’s with 6 months direct care in mental health/substance use.
• OK RN license with behavioral health experience may apply.

Responsibilities

• Provide integrated, accessible services addressing physical and behavioral health needs.
• Engage families and support systems to assist treatment goals.
• Develop care plans based on assessments of symptoms, history and goals.
• Assist clients with eligibility for benefits, housing, transportation, and employment.
• Coordinate care across treatment settings and prepare for primary care visits.
• Maintain timely clinical records and ensure confidentiality and compliance.
