The Windsor Laurelwood Center for Behavioral Medicine in Willoughby, Ohio, is seeking a Full Time Step Down Navigator (Social Work) to meet eligible patients and discuss benefits of outpatient services as part of their continuum of care.

The role requires a Master’s degree in Social Work or related field, Ohio licensure (LSW/LISW/LPC/LPCC), and at least two years in mental health or substance abuse treatment, with strong communication and discharge planning skills.

Competenze

• Master's degree in Social Work, counseling, or a related mental health field from an accredited college or university.
• Must possess an LSW, LISW, LPC or LPCC issued by the State of Ohio.
• Two years previous experience in a mental health or substance abuse treatment facility with direct experience with group therapy, crisis intervention, and treatment planning.
• Excellent verbal and written communication skills; diplomacy and discretion; ability to prioritize multiple projects.

Mansioni

• The Step Down Navigator is responsible for meeting all eligible patients to discuss the benefits of outpatient services as part of their continuum of care treatment.
• Communicate eligibility to key members of senior leadership and associated departments and will be responsible for routinely collecting and entering data, specific to outpatient eligibility, into Midas for trending report purposes.
• Have a thorough understanding of the Step-Down Playbook and inpatient workflow as it pertains to case management and discharge planning.
• This individual can facilitate groups, assist with continued stay reviews and float to the inpatient units as needed.
