Stormont Vail Health is seeking a Behavioral Health Admissions & Nurse Navigator to coordinate patient access, admission, and navigation across the behavioral health continuum. Qualifications

• Strong psychiatric assessment skills across lifespan.
• Experience with behavioral health intake/admissions processes.
• Ability to coordinate care across multidisciplinary teams.

Responsibilities

• Lead behavioral health admission process with a centralized access model.
• Perform comprehensive psychiatric assessments (child to geriatric) to determine level of care.
• Facilitate admissions to inpatient and outpatient services based on clinical criteria.
• Coordinate alternative placements when admission is not appropriate or capacity is limited.
• Identify and address barriers to admission and access to care.
• Ensure intake/admission documentation is complete and compliant with regulations.
• Guide patients and families through the healthcare system from intake to episode of care.
• Coordinate services with psychiatrists, nurses, social workers, case managers and community providers.
• Connect patients to community resources and educational programs.
• Track patient progress and outcomes across the continuum of care.
• Triage calls and conduct initial intakes including clinical and insurance information.
• Collaborate with various teams to ensure efficient patient flow.
• Oversee appointment scheduling and ensure timely throughput.
• Verify insurance benefits and obtain prior authorizations when needed.
