Neighborhood Health Association in Northwest Ohio is seeking a Patient Navigator to coordinate care, promote health literacy, and help patients access community resources. The role partners with providers, care teams, and patients to improve health outcomes and satisfaction.

Responsibilities include care planning, addressing barriers to care, and ensuring accurate documentation within the EHR. A bachelor’s degree in a related field and experience in care coordination are preferred.

Qualifications

• Bachelor's degree in Social Work, Public Health, Healthcare Administration, Human Services, or related field preferred.
• Experience in care coordination, case management, patient advocacy, social services, or healthcare navigation preferred.

Responsabilités

• Coordinate patient care, referrals, and follow-up visits to ensure continuity of care.
• Educate patients and families about diagnoses, treatment plans, insurance coverage, and resources.
• Develop collaborative care plans based on provider recommendations and patient goals.
• Identify barriers to care (transportation, finances, housing, language) and connect patients with community services.
• Document patient interactions accurately within the EHR and maintain HIPAA compliance.
