Job summary

Lutheran Social Services of Southern California is seeking a Community Supports (CS) Case Manager I–III to oversee case management for victim intervention and housing navigation programs. The role focuses on client-centered care, ensuring access to housing resources and related social services.

Responsibilities include intake, assessment, care planning, linkage, and coordination with community resources, with emphasis on compliance, documentation, and trauma-informed services for clients.

Qualifications

• Education requirements include a bachelor’s degree in social work or related field or equivalent combined experience.
• Experience in case management or social services is required or preferred depending on level.
• Ability to maintain HIPAA privacy and professional boundaries in client services.

Responsibilities

• Oversee case management for housing navigation and victim intervention programs.
• Ensure timely documentation and chart compliance with program policies.
• Coordinate with community resources and housing services to meet client needs.
• Provide culturally responsive care and maintain client confidentiality.
