Job summary

The People Concern is seeking an ICMS Case Manager to support housing retention efforts for clients experiencing homelessness in the Los Angeles area. This onsite role requires coordinating with housing and health services, while applying harm reduction and trauma-informed care principles.

You will manage client intakes, assessments, and tailored plans, aiming to stabilize housing and improve community integration through collaboration with DMH, DHS, and local partners.

Qualifications

• Must have High School Diploma or GED or Equivalent.
• CA Driver's license valid; reliable car and auto insurance.
• Willing to transport Clients in personal vehicle; acceptable driving record.

Responsibilities

• Maintain a caseload of about 20 high-acuity Clients (up to 40).
• Develop relationships to support independence and health.
• Coordinate with DMH and DHS programs for collaboration.
• Assist in screening, intake, and enrollment of Clients.
• Complete biopsychosocial assessments and treatment planning.
• Prepare quarterly collaborative goal plans with Clients.
• Maintain accurate progress notes in electronic databases.
• Ensure all Client documents are complete and confidential.
• Coordinate appointments and transportation for access to care.
• Transport Clients to agencies and providers as needed.
• Teach life skills such as budgeting and household management.
• Communicate with property managers and partners.
• Participate in weekly staff meetings and trainings.
• Adapt workload per funder expectations.
• Perform other duties as assigned.
