Job summary

The People Concern in Downtown Los Angeles is seeking a Clinical Case Manager to provide intensive field-based case management for clients pursuing permanent housing. You will work with a diverse caseload, assess mental health needs, link to services, and coordinate resources within a trauma-informed, harm-reduction framework.

The role requires MSW/MFT/LPC eligibility and experience with homelessness and behavioral health.

Qualifications

• MSW, MFT or LPC with BBS eligibility or eligibility within 2 months.
• Registered with the Board of Behavioral Sciences or ability to do so.
• Experience with homelessness, mental health and substance use issues.
• Valid CA driver's license with reliable car.
• Typing speed 30 WPM preferred.

Responsibilities

• Provide intensive field-based case management to a caseload.
• Assess mental health needs and link to services.
• Coordinate medical, mental health, and vocational resources.
• Maintain client files and HIPAA confidentiality.
• Travel to multiple locations in the greater Los Angeles area.
