Job summary

The People Concern is seeking an ICMS Case Manager for the 1603-Housing Retention program in Woodland Hills, CA. This on-site role supports homeless clients through housing placement and retention with trauma-informed care and harm-reduction principles.

You will manage a caseload, coordinate with local agencies, and help clients access health and social services while teaching independent living skills for successful transition to permanent housing.

Qualifications

• High School Diploma or GED required; Bachelor’s degree preferred or equivalent experience.
• Two years’ experience with homeless individuals or related field recommended; driving required.
• CA driver’s license, reliable car, insurance, and valid driving record required.

Responsibilities

• Maintain a caseload of approximately 20 high-acuity Clients (up to 40 for low acuity).
• Coordinate with LA County DMH and DHS to align treatment plans.
• Assist with intake, enrollment, and orientation to program policies and goals.
• Complete biopsychosocial assessments and formulate individualized treatment plans.
• Arrange transportation and coordinate with providers to improve access to services.
• Document progress notes and maintain confidential records in accordance with HIPAA.
