Job summary

The Case Manager at Church World Service will provide direct case management and holistic support to clients facing complex challenges, including housing, healthcare, and legal assistance. You will conduct intake assessments, develop service plans, document progress, and advocate within community systems to ensure trauma-informed, culturally responsive care.

Qualifications

• Bachelor's degree in social work or related field or equivalent experience.
• 3–4 years of case management/direct service experience in nonprofit/humanitarian settings preferred.
• Excellent communication, documentation, and interpersonal skills.
• Knowledge of trauma-informed, strength-based, culturally responsive care.

Responsibilities

• Conduct intake assessments to determine client needs and eligibility.
• Develop individualized service plans with measurable goals and timelines.
• Provide ongoing case management and progress monitoring.
• Facilitate access to housing, healthcare, employment, education, and legal support.
• Advocate for clients with external agencies to secure resources.
• Maintain confidential case files and ensure data privacy.
• Coordinate referrals and collaborate with community partners.
• Participate in trainings to maintain best practices in trauma-informed care.
