Lutheran Services Florida (LSF) seeks a Case Manager Intensive to deliver trauma-informed, intensive case management and supportive services to vulnerable clients. The role involves crisis intervention, safety planning, and coordinating referrals to ensure culturally appropriate care, with supervision of interns and volunteers as part of a collaborative team.

Qualifications

• Bachelor's degree required in social work or related field.
• Two years of case management or social services experience preferred.
• Experience with refugees/immigrants and cross-cultural work preferred.

Responsibilities

• Provide trauma-informed intensive case management and supportive services to clients.
• Conduct weekly in-person trauma support focusing on crisis intervention and safety planning.
• Develop goal-oriented self-sufficiency plans addressing long- and short-term needs.
• Maintain comprehensive case files and documentation per LSF policies and procedures.
• Coordinate referrals and follow-up to ensure services are linguistically and culturally appropriate.
• Train and supervise program interns and volunteers.
• Participate in stakeholder meetings and trainings centered on trauma-informed care and community support.
• Prepare program reports as required.
