Job summary

Lutheran Social Services of Southern California is seeking a Community Supports Case Manager I–III to oversee case management for Victim Intervention and housing resources, focusing on trauma-informed client-centered care. The role emphasizes access to housing services and coordination with community resources.

Under supervision, you will document, monitor charts, ensure HIPAA compliance, and contribute to program quality.

Qualifications

• Education: High School Diploma with 2 years of experience.
• Bachelor’s degree with 1 year of experience.
• Master’s degree in social work or related field with internship plus six months in case management.
• Ability to read, analyze, and interpret general business materials.
• Excellent communication and interpersonal skills.

Responsibilities

• Provide culturally responsive, empathetic care while serving clients in housing navigation and other social services.
• Assess client eligibility and needs for housing and related services.
• Deliver case management: intake, assessment, care planning, linkage, and coordination.
• Assist clients in accessing emergency shelters, permanent supportive housing, and other housing resources.
• Document monthly in client charts, ensure policy compliance and timely reporting.
• Maintain confidentiality and HIPAA compliance.
