Job summary

The People Concern is seeking an ICMS Case Manager to support two program sites in Woodland Hills, CA, balancing caseloads and coordinating housing stabilization services for Clients experiencing homelessness. The role emphasizes harm reduction, trauma-informed care, and community integration with a strong focus on independent living skills and ongoing client support.

Qualifications

• High School Diploma or GED.
• Associate degree preferred; or 2 years' homeless-services experience.
• CA Driver's license and reliable vehicle required.
• Ability to transport Clients in personal vehicle.

Responsibilities

• Maintain a caseload of high-acuity Clients for intensive case management.
• Coordinate housing opportunities and access to health services.
• Collaborate with DMH and DHS programs for coordinated care.
• Prepare assessments, treatment plans, and progress notes in databases.
• Ensure confidentiality and paperwork compliance with funders.
