Job summary

The People Concern is seeking a Case Manager for onsite work at our housing programs in Los Angeles. You will support clients to achieve housing stability, self-sufficiency, and community integration using harm reduction and trauma-informed practices.

You will manage a caseload, collaborate with DMH and DHS partners, and coordinate services to improve health outcomes and access to resources. Strong communication and documentation are essential.

Qualifications

• Bachelor's degree in social work or related field required or equivalent experience.
• Demonstrated knowledge of issues faced by a culturally diverse population.
• Valid CA driver’s license, reliable car, auto insurance, and acceptable driving record.
• Willing and able to transport Clients in personal vehicle.

Responsibilities

• Maintain a caseload of approximately 20 high-acuity Clients for intensive case management services (up to 40 if low acuity).
• Develop relationships with Clients to facilitate independence and improve health outcomes.
• Collaborate with LA County DMH and DHS programs to coordinate treatment teams.
• Engage Clients with harm-reduction and trauma-informed care within housing-first guidelines.
• Assist in screening, intake, enrollment, and orientation to program policies.
• Complete biopsychosocial assessments and functional needs assessments to guide individualized planning.
• Write quarterly collaborative goal plans to improve quality of life and health outcomes.
• Maintain accurate progress notes and document in electronic databases.
• Ensure Client documents are complete, accurate, and filed per guidelines.
• Maintain confidentiality of Client information per HIPAA and guidelines.
• Coordinate appointments, transportation, and follow-up services to improve access to care.
• Transport Clients to agencies/providers in the LA area to support housing retention.
• Model life skills and support independent living, budgeting, and health maintenance.
• Communicate with property managers and third-party partners.
• Participate in weekly staff meetings, supervision, and trainings.
• Adapt workload as directed by supervisor to meet funder expectations.
