The People Concern is seeking an ICMS Case Manager in Los Angeles to support housing retention and client self-sufficiency. This onsite role partners with Clients transitioning from homelessness, assessing barriers and coordinating services to secure and maintain housing.

You will work within the Permanent Housing program, applying harm‑reduction, trauma‑informed care, and collaboration with local agencies to meet individualized needs.

Qualifications

• High School Diploma or GED required.
• Valid CA driver's license, reliable car, auto insurance.
• Willing to transport Clients in your personal vehicle.
• Able to work in a high tolerance Harm Reduction model with Clients experiencing multiple barriers.

Responsabilités

• Maintain a caseload of approximately 20 high‑acuity Clients for intensive case management services.
• Develop relationships with Clients with a focus on independence and health.
• Coordinate with DMH and DHS programs to enhance coordination within the Clients' treatment team.
• Engage Clients with an approach emphasizing harm reduction and trauma‑informed care.
• Assist in screening, intake, and enrollment of Clients and program orientation.
• Complete biopsychosocial assessments to guide treatment planning.
• Write progress notes and manage documentation in electronic databases.
• Coordinate transportation and appointments to improve access to services.
• Teach life skills and support independent living as Clients transition to housing.
