The People Concern in North Hollywood, CA is seeking a Case Manager for Project Based Housing to support clients facing homelessness with housing stability using harm reduction, housing first, and trauma-informed care approaches.

You will manage a caseload of about 20 high-acuity clients, coordinate with LA County DMH and DHS, assist with benefits, transport clients to services, and help teach independent living skills for successful transition to permanent housing.

Qualifications

• High School Diploma or GED required.
• Associates or Bachelor's degree preferred; or 2 years’ homeless services experience.

Responsabilités

• Maintain a caseload of approximately 20 high-acuity Clients for intensive case management.
• Coordinate services with LA County DMH and DHS programs to improve treatment.
• Assist Clients with benefits and transportation to primary health and mental health services.
• Directly transport Clients in personal vehicle to agencies and providers in LA area.
• Document progress notes in electronic databases and maintain confidentiality.
