Job summary

The Center for Transforming Lives is seeking a Transitional Housing Case Manager to provide intensive trauma-informed case management to unaccompanied female victims of crime/trauma experiencing homelessness. You will build safe relationships, support housing search and placement, and work to prevent returns to homelessness by partnering with landlords and service providers.

You will conduct comprehensive assessments, collaborate with MDTs, and help families plan to achieve housing stability,

Qualifications

• Bachelor's degree in social work or related field; Master’s preferred and experience with vulnerable populations is a plus.
• Trauma-informed approach is required across all services.
• Experience coordinating with landlords, housing authorities, and community programs.
• Strong written and verbal communication skills and cultural sensitivity.
• Knowledge of housing resources, tenancy rights, and program eligibility.

Responsibilities

• Plan for discharge and participate in MDT reviews to assess readiness.
• Provide intensive trauma-informed case management and needs assessment.
• Assist families with housing search and unit placement in reachable areas.
• Cultivate relationships with landlords and agencies to expand housing options.
• Conduct Housing Quality Standard inspections and coordinate move-in tasks.
• Apply trauma-informed strategies with women and children served.
• Complete assessments and use findings to inform service planning.
• Coordinate service plan meetings and document progress and safety plans.
