Job summary

Mental Health Association - Western MA is seeking a dedicated Case Manager to support individuals transitioning from homelessness into independent housing within a supportive housing setting. You will provide trauma-informed, person-centered services including assessments, plan development, and ongoing support to improve housing stability, income, and coping skills.

Qualifications

• High school diploma or equivalent required.
• Minimum of six months of relevant experience required.
• Bilingual English/Spanish preferred.
• Must be 21+ and able to obtain CPR and First Aid certifications.

Responsibilities

• Coordinate documentation for program entry and required IDs.
• Complete adult assessments and service plans.
• Develop individualized action plans with participant input.
• Implement IAPs and track progress.
• Develop/monitor safety plans with participants and team.
• Assist transition to stable housing and advocate for benefits.
• Support employment, training, or education pursuits.
• Educate on public benefits and community resources.
• Promote community integration and skill development.
