Job summary

The Center for Transforming Lives is seeking a Transitional Housing Case Manager to provide intensive trauma-informed case management to unaccompanied female victims of crime/trauma experiencing homelessness. You will build safe relationships, support housing search and placement, and work to prevent returns to homelessness by partnering with landlords and service providers.

Qualifications

• Bachelor's degree in social work or related field; Master’s preferred and experience with vulnerable populations is a plus.
• Trauma-informed approach is required across all services.
• Experience coordinating with landlords, housing authorities, and community programs.
• Strong written and verbal communication skills and cultural sensitivity.
• Knowledge of housing resources, tenancy rights, and program eligibility.

SUMMARY

The Transitional Housing Case Manager has the primary responsibility to provide intensive case management and supportive services to unaccompanied female victims of crime/trauma experiencing homelessness utilizing a trauma-informed, two-generation approach. Build safe, trusting relationships, and exercise appropriate boundaries and interactions with program participants. Promote long-term housing stability and prevent returns to homelessness by cultivating and maintaining ongoing relationships with property managers/landlords. Collaborate with program participants in housing search and placement activities, including selecting units in close proximity to school and work, to the greatest extent possible, mitigating potential transportation and or employment barriers.

RESPONSIBILITIES

• Plan for discharge upon program admission. Actively participate in MDT meetings to review program participant strengths, needs, service plan goals, and program extensions. Collaborate to evaluate discharge readiness on an ongoing basis.
• Provide intensive, trauma-informed case management services based on the complexity of need. Assist families with goal setting, needs assessment, information and referrals.
• Provide program participants a variety of property options in consideration of the program participant’s ability to financially sustain the unit post-discharge. Collaborate with program participants in housing search and placement activities.
• Cultivate and maintain ongoing working relationships with landlords, local housing authorities, and other pertinent community agencies; act as a liaison when needed. Utilize Motivational Interviewing techniques to assist program participants with resolving ambivalence and promoting engagement and motivation.
• Complete a Housing Quality Standard Inspection and lead-based paint (LBP) assessment in accordance with Agency Policy and Procedure and HUD regulations. Schedule and coordinate move-in; complete all necessary housing documents; and attend lease signing within 30 days from the date of admission. Orient families to program expectations and housing requirements as indicated in the lease agreement and Housing Assistance Payment Contract (HAP).
• Demonstrate a deep understanding of trauma and its impact on adults and children as evidenced by the implementation of trauma-informed care principles.
• Attend and actively participate in trauma-focused clinical consultation. Apply suggested trauma-informed strategies and interventions consistently to women and children served.
• Complete trauma-informed, strength-based assessment activities (i.e., Transforming Lives Scale, ACE Questionnaire, etc.) and comprehensive assessments in strict adherence to related program policies and procedures. Use assessment findings to inform service planning.
• Involve program participants in all aspects of service planning including regular updates. Coordinate and facilitate timely service plan meetings. Develop measurable service plan goals and interventions. Develop safety plans as deemed appropriate throughout the course of service provision.
• Cross-collaborate with internal and external programs to refer and link program participants and their children to need-driven supportive services. Establish connections with community service providers to develop and enhance program resources.
• Intervene and immediately resolve conflicts and concerns. Escalate such issues to the Residential Services Lead Clinical Case Manager and document them in an incident report, when needed.

EDUCATION AND EXPERIENCE

Bachelors’ degree in social work or related field; Master’s degree is preferred and two (2) years of experience working with vulnerable/at-risk populations. The ability to work with and recognize the special needs of individuals and families experiencing homelessness and knowledge of a wide range of community resources.

KNOWLEDGE, SKILLS, ABILITIES

• Knowledge and experience in trauma-informed care and evaluation and documentation of services with a clinical approach; excellent written and oral communication skills; excellent interpersonal skills and the ability to work with all levels of management, staff, women and children.
• Case management and case coordination skills. Knowledge of housing resources and community programs and how to access them; mainstream benefits programs and eligibility requirements; basic landlord-tenant rights and responsibilities and tenancy skills and or supports.
• Effective communication. Problem-solving, conflict resolution and cultural sensitivity when working side by side with families. Attention to detail and the ability to prioritize and execute multiple projects under demanding deadlines.
• Knowledge of Windows operating system and Office products. Ability to learn and use any other software programs needed in the operations of CTL programs.
• Valid driver’s license and willingness to travel in the community.
