Job summary

Center for Transforming Lives is seeking a Rapid Rehousing (RRH) Case Manager to provide intensive community-based case management and supportive services to individuals and families experiencing homelessness. You will promote housing stability by building relationships with landlords, coordinating housing options, and applying trauma-informed care to guide planning and discharge readiness.

Collaborate with the MDT to evaluate progress, develop post-discharge housing goals, and support families

Qualifications

• Bachelor's degree in social work or related field; master’s degree preferred.
• Two years of experience working with vulnerable/at-risk populations preferred.
• Ability to work with diverse populations experiencing homelessness and knowledge of community resources.

Responsibilities

• Collaborate with individuals and families in housing search and placement activities; maintain relationships with landlords and housing authorities.
• Complete a Housing Quality Standard Inspection and lead-based paint assessment; coordinate move-in and lease signing within 30 days.
• Re-determine rental assistance eligibility per policies and CoC procedures.
• Apply trauma-informed, strength-based assessments to inform service planning.
• Participate in trauma-focused clinical consultations and apply trauma-informed strategies.
• Involve families in service planning; develop measurable goals and safety plans; participate in MDT meetings.
• Refer and link families to need-driven supportive services; connect with community providers.
• Provide intensive, trauma-informed case management based on needs; assist with goal setting and referrals.
• Intervene and escalate conflicts; document incidents as needed.
