Qualifications

• Bachelor's degree in Social Work, Sociology, Psychology, or related field.
• 1+ years of experience in a social service agency or customer service environment.
• Experience with mental health and/or victims of domestic violence population is a plus.
• Must maintain regular and acceptable attendance.
• Driver's license with a clear record.
• Daily use of a vehicle.

Responsibilities

• Develops trusting relationships with program participants using a trauma-informed approach.
• Uses Housing First model to connect participants with housing and community resources.
• Maintains relationships with landlords and community partners to obtain housing.
• Creates and updates service plans with goals and follow-ups.
• Documents weekly case notes and HMIS entries; prepares reports.
• Provides crisis assessment and coordinates additional services as needed.
• Collaborates with multidisciplinary teams to promote long-term housing stability.
• Delivers life skills training and budgeting support to residents.
