Job summary

The Whole Person Care Clinic is seeking a Case Manager to provide direct, trauma-informed case management to individuals experiencing homelessness and Recuperative Care participants. You will bridge acute medical care, behavioral health, housing stability, and community support services with compassion and advocacy.

Responsibilities include intake assessments, developing service plans, coordinating care across providers, and ensuring accurate documentation in HMIS to support seamless discharge

Qualifications

• Bachelor’s degree in social work, psychology, public health, or related field (or equivalent experience).
• Minimum 1–2 years of case management or social services experience in homeless services or medical respite settings.
• Knowledge of local community resources and social service systems.
• Ability to work with individuals experiencing homelessness, mental illness, substance use, and complex medical conditions.
• Proficiency with basic computer applications and electronic case management systems; valid driver's license if required.

Responsibilities

• Conduct comprehensive intake assessments to identify housing, medical, behavioral health, social, and basic needs.
• Develop individualized service plans with measurable goals in collaboration with clients and staff.
• Provide regular case management check-ins and adjust plans as needs evolve.
• Coordinate care with outside providers and ensure timely communication and documentation in HMIS or similar system.
• Coordinate safe discharge planning for Recuperative Care patients transitioning to the community.
