Job summary

Wesley Health Centers - JWCH Institute, Inc. is seeking a dedicated Case Manager to deliver holistic case management for patients, coordinate services, and advocate for access to medical, housing, and support programs.

The role emphasizes trauma-informed care, harm-reduction, and seamless collaboration with clinicians and social services to improve outcomes. The position requires coordinating care plans, documenting progress, and adhering to DMH contract standards while engaging in community

Qualifications

• AA or Bachelor’s degree in Social Work, Sociology, or related field; 1 year of care coordination experience preferred.
• Experience with EHR systems, especially Next-Gen, preferred.
• Bilingual English/Spanish abilities preferred.

Responsibilities

• Provide comprehensive case management including intake, assessment, and care plan development.
• Coordinate medical, social, and housing services and follow up on referrals.
• Advocate for patients using trauma-informed care and harm-reduction approaches.
• Maintain patient records and submit required reports in a timely manner.
• Collaborate with multidisciplinary teams and participate in program trainings.
