The role involves conducting intakes, coordinating services, and maintaining comprehensive documentation while collaborating with a multidisciplinary team to support client goals.

Qualifications

• Bachelor's degree in a social services field or related area.
• Three to five years of case management experience preferred.
• Bilingual English/Spanish preferred.

Responsibilities

• Completes intakes for new clients and orients them to program rules.
• Collaborates with multidisciplinary staff to monitor client progress.
• Develops psychosocial assessments to address client needs.
• Assists clients with housing applications and service referrals.
• Maintains case records and documentation in standard databases.
