Job summary

Colorado Coalition for the Homeless in Denver seeks a Case Manager I to collaborate with teams to promote long-term solutions for clients experiencing homelessness. You will develop care plans, coordinate services, monitor progress, and advocate for access to benefits and housing resources in a trauma-informed, client-centered approach.

Qualifications

High School Diploma or GED required.
Bachelor’s degree in Psychology, Social Work or related field preferred.

Responsibilities

Manage a caseload of clients with high-service needs and disabilities.
Coordinate services and advocate for clients across health and housing resources.
Maintain comprehensive case documentation and data in the EHR and related systems.
Provide crisis intervention, de‑escalation, and referrals as needed.
Collaborate with community partners and health providers to ensure seamless care.
