Colorado Coalition for the Homeless in Denver seeks a Case Manager I to collaborate with teams to promote long-term solutions for clients experiencing homelessness. You will develop care plans, coordinate services, monitor progress, and advocate for access to benefits and housing resources in a trauma-informed, client-centered approach.

Competenze

• High School Diploma or GED required.
• Bachelor’s degree in Psychology, Social Work or related field preferred.

• Manage a caseload of clients with high-service needs and disabilities.
• Coordinate services and advocate for clients across health and housing resources.
• Maintain comprehensive case documentation and data in the EHR and related systems.
• Provide crisis intervention, de‑escalation, and referrals as needed.
