Job summary

Resources for Human Development is seeking a compassionate Case Manager to provide community-based support to individuals experiencing homelessness and behavioral health challenges. You will work with a multidisciplinary team to deliver person-centered, recovery-oriented care in the program0dated community setting.

The role emphasizes connecting participants to resources, supporting recovery goals, and advocating for services that promote independence and stability within the service area.

Qualifications

• Bachelors degree preferred.
• Direct care experience in behavioral health/addiction/IDD preferred.
• Experience with homelessness is preferred.
• Valid drivers license required.

Responsibilities

• Collaborate with a multidisciplinary team to support participants from a holistic, values-based perspective.
• Develop and update individualized recovery plans.
• Coordinate care with clinical, medical, and community stakeholders.
• Provide community-based services including resource connection and life skills.
• Transport participants to appointments as needed.
• Complete assessments, documentation, and progress notes; ensure compliance with policies.
